Mobile Primary Care
New patient admissions, annual wellness visits, preventive care, chronic disease management, acute illness evaluations, medication management, lab review, referrals, advance care planning, and care coordination.
Televisit Visits
Available for new and established patients with acute illnesses and select concerns, including cold/flu, COVID-19 symptoms, sore throat, sinus infections, UTIs (when appropriate), pink eye, rashes, allergies, medication refills (when appropriate), follow-up visits, lab review, chronic disease management, and caregiver consultations. Same-day appointments may be available based on NP availability.

Mobile Wound Care
Pressure injuries, diabetic foot ulcers, venous/arterial ulcers, surgical wounds, skin tears, traumatic and chronic wounds, dressing changes, conservative debridement when appropriate, infection monitoring, education, and home health coordination.
Transitional Care Managemnet
Post-hospital follow-up, medication reconciliation, discharge review, chronic disease management, coordination with specialists and home health, education, and monitoring to reduce readmissions.
